Bill Gates committed the major mistake by issuing a companywide memo that said every project and product in Microsoft had to incorporate the Internet. This happened when Bill was having his infamous Think Weeks where he locked himself in a retreat for almost two weeks to indulge himself in the thinking process. Without any calls or mails Bill realized that he totally missed the internet opportunity, which resulted in the mess up.

12.  Lee Iacocca, Former Chairman of  Chrysler:

Lee Iacocca said that hiring of Bob Eaton from the General Motors Corp in the year 1992, as the President of Chrysler and Chairman –in-waiting was his biggest mistake ever done in his professional life. He also said that, In spite of him, it would have been better if I would have hired Louis Hughes or John F. Smith Jr who were the high ranked GM Executives at that time.. He said, ‘Eaton was always a staff guy as he never ran a division’.  Lee had revealed this mistake in an interview with Wards Auto in the year 2003
